IMNormal renal function Adult, childn & older infant 15 mg/kg/day divided into 2 or 3 equal doses administered at equally divided intervals. Patient w/ heavier wt Max: 15 g/day. Newborn Loading dose: Initially 10 mg/kg followed w/ 7.5 mg/kg every 12 hr. Usual duration of treatment: 7-10 days. Max total daily dose: 15 mg/kg/day. Uncomplicated UTI 250 mg bd. IV Same as IM dose. Dose adjustment should be made according to age & symptoms. Adult & ped Administered by IV infusion over 30-60 min period. Infant Should receive 1-2 hr infusion.
History of deafness due to aminoglycosides. Observe patients who are not taking adequate oral feedings or those receiving parenteral feeding, elderly & who are not in good general condition. Contains Na metabisulfite & may cause allergic-type reactions. Renal & liver disorder. Pregnancy & lactation. Newborns & prematures.
Enhanced nephrotoxic effects of blood substitutes (eg, dextran & NA alginate). Resp depression due to curare-like effect may occur w/ anesth or muscle relaxants. Nephrotoxicity &/or ototoxicity may be enhanced w/ loop diuretics (eg, ethacrynic & furosemide).
